Licensor observed one volunteer named Jason that was listed as being employed and did not have TB test confirmed, and no longer works there and will need to be end dated. Merit page is not being managed in order to verify all staff being in compliance with required trainings for their roles, and on inspection date this was not in compliance and files are incomplete to support what is not entered in Merit at this time or incomplete trainings to meet Merit requirments for required roles.
